The Oxford Academic Health Science Network (Oxford AHSN) is establishing a Polypharmacy Community of Practice within the Frimley Integrated Care System (ICS). This is part of the national AHSN Network Polypharmacy Programme: Getting the Balance Right.
The Frimley ICS Polypharmacy Community of Practice is open to all interested health or social care professionals,
representatives from healthcare groups, patients or individuals from patient groups or organisations within the Frimley ICS area.
This Community of Practice will:
- foster a system-wide culture of collaboration, learning and innovation to reduce problematic polypharmacy for those most at risk from harm;
- provide a system-wide forum for discussion and connect colleagues across the Frimley ICS who are interested in tackling problematic polypharmacy.
At the first Community of Practice meeting on Friday 16 September (1300-1400), Dr Adrian Hayter, GP and National Clinical Director for Older People and Integrated Person Centred Care, NHS England, will speak on the risks of harm caused by polypharmacy in the older population.
