Frimley GP 2024-25 Pharmacy & Medicines Optimisation Scheme

Each year Frimley ICB agree an optional Medicines Optimisation Scheme with GPs across Frimley with the aim of improving outcomes achieved from medicines use.  This includes reducing admissions relating to adverse reactions to medications and improving long term condition management, improving use of community pharmacy services to reduce use of urgent care services and general practice, and to achieve more cost-effective prescribing and budget achievement.

Practices choose 5 out of a possible 7 elements to complete and receive a payment if the work is undertaken.  One of the core elements this year (which means that all participating practices are asked to complete this element) is community pharmacy integration.

The community pharmacy Integration element is for practices to:

Improve day-to-day communication between pharmacies and practices as well as improve the Pharmacy First service. (Practices to arrange to meet regularly with their local pharmacies.)

Make regular referrals to the Pharmacy First service on a day to day basis for appropriate individuals (from at least 1st September 2024 to 31st March 2025 for this element to be successfully completed).

Community pharmacy – What to expect

You may therefore, as part of this incentive scheme, be contacted by your local practices, to discuss, at your convenience, the Pharmacy First service and ways to improve the overall service for yourself, the practice and Frimley residents who access the service with an aim to increase the referral numbers into your pharmacy.

The practice will be responsible for contacting you, and it is the practice’s responsibility to draw up and monitor any agreed action plan between the pharmacy and the practice.

This is a GP incentive scheme so you are not expected to lead on any elements of the work identified/required as part of this scheme.
